Spiritual Advisor

Shift: Part-time (2-3 days per week)

JOB FUNCTION:

Auduzhe is seeking a part-time Spiritual Advisor to join our Indigenous-led, interdisciplinary primary health care team. The Spiritual Advisor will provide dedicated spiritual care to First Nations, Inuit, and Métis (FNIM) community members, facilitate cultural programming and ceremonies, and offer cultural guidance that fosters relationships and promotes healing. This role works collaboratively with the interprofessional team to support holistic health and wellness.
As part of the care team, the Spiritual Advisor will contribute to the development and delivery of culturally grounded programming while providing compassionate, one-on-one spiritual support. The role involves collaboration with staff on care planning and case management, and requires strong, empathetic communication. This position is pivotal in strengthening the spiritual environment and promoting overall wellbeing across Auduzhe.

Community Health Worker – Auduzhe Mino Nesewinong

Shift: 35hr a week

Closing: May 7th, 2026

Location: 22 Vaughan Road, Toronto (Auduzhe Mino Nesewinong Clinic)

JOB FUNCTION:

Roles and Responsibilities includ:
▪ Provide culturally safe care to First Nations, Inuit, and Métis community members in Tkaronto
▪ Assist in locating culturally appropriate community resources by liaising with community groups and other health/social service agencies to provide referral information for clients
▪ Facilitate referrals to other community health center professionals to ensure a care continuum of health and social services
▪ Work closely with Auduzhe’s medical providers to ensure community members have comprehensive and coordinated care plans
▪ Provide short term crisis prevention/response, de-escalation, and stabilization through instrumental support and referrals to culturally appropriate crisis services
▪ Facilitate transportation and/or accompany clients to medical appointments, court, community programs and other appointments as needed
▪ Establish trusting relationships with FNIM community members and families
▪ Ensure clients are integrated into the appropriate services both within Auduzhe Mino Nesewinong and wider community as required
▪ Document on each client contact including services provided/offered including referrals
▪ Collaborate with one’s service team and other teams across the organization to identify and provide health education to target groups
▪ Facilitate client referrals to relevant resources
▪ Provide case support and education to clients

Nurse Practitioner (Indigenous Health)

Shift: 35 hrs/week (Daytime hours, occasional weekend and evening)

Location: Auduzhe Mino Nesewinong (22 Vaughan Road, Toronto) and Na-Me-Res Shelter (14 Vaughan, Toronto)

JOB FUNCTION:

The Nurse Practitioner (NP) will provide culturally safe, comprehensive primary care and episodic services across two locations: Auduzhe Mino Nesewinong Primary Care Team at 22 Vaughan Road and Na-Me-Res (Native Men’s Residence) Emergency Shelter at 14 Vaughan Road.
This role includes supporting clients at Na-Me-Res Emergency Shelter for 2–3 days per week, with remaining time spent at Auduzhe within their primary care team.
The NP will deliver holistic care to individuals and families, focusing on relationship-building, social medicine, and primary prevention. Responsibilities include conducting health assessments, managing complex health and social conditions (e.g., sexual health, chronic disease, mental health, substance use), and promoting early detection and prevention strategies.
Working collaboratively with an interdisciplinary team, the NP will provide outreach and on-site episodic, transitional, primary care, and public health services, ensuring compliance with PHIPA and Indigenous privacy standards. The NP will support shelter and transitional housing initiatives and respond to emerging needs as required. This position reports jointly to the Medical Director and the Shelter Manager.
